New Patient Registration

You will be asked to complete registration forms for each individual wishing to register alongside a short questionnaire. On completion of both forms over 16’s are required to produce two forms of identification one of which must be photographic along with confirmation of address e.g.utility bill. 

As a Practice, we assume that patients have consented to the disclosure of personal health information, to the extent that it is necessary for the purposes of the operational management of the Practice and related facilities concerned with the provision of care. 

The Practice complies with the Data Protection Act 1998.

Practice Area

Our catchment area covers a coastal strip extending from Spey Bay to Cullen and incorporating the town of Buckie (population 8,000) as well as the villages of Portgordon, Findochty, Portknockie and Cullen.

Registration With The Practice Online

If you wish to register as a new patient with the practice please complete:

When you have completed all of the above, you can then email your completed registration paperwork to gram.ardachadministrator@nhs.scot along with your two forms of identification (one form of photographic identification, for which you will require to take a photo of yourself holding this and one utility/letter with your current address).  

If you do not have access to the internet you can hand in your completed forms in person with the relevant identification as stated above to the reception staff.  We are sorry, but due to legislation this form has to be handed in to the surgery by each individual.

Non-English Speakers

Temporary Registration

When you have completed all of the above, you can then email your completed temporary registration paperwork to gram.ardachadministrator@nhs.scot with a form of photographic identification, for which you will require to take a photo of yourself holding this when emailing this in. 

If you are ill while away from home and need to receive treatment for an acute medical emergency you can register as a temporary patient. 

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or local area where you are already registered.

If you have run out of medication while away from home, and are registered with a Scottish GP practice, you can attend any community pharmacy who will be able to help you without being seen by a GP.
