Practice Area

Our catchment area covers a coastal strip extending from Spey Bay to Cullen and incorporating the town of Buckie (population 8,000) as well as the villages of Portgordon, Findochty, Portknockie and Cullen.

Registering with a GP practice

To register permanently, you’ll need to complete a registration form. You can download the forms below or, alternatively, you may attend the practice to collect and complete registration forms:

If you wish to register a newborn baby please complete the Regsitration and Medical Questionnaire for New Born Baby forms, you will also require to hand in the letter you receive from the Registrars.

When you register with us, it’s recommended that you provide information to confirm your:

  • identity
  • address

This means you can be matched to your existing NHS records. You shouldn’t be delayed or refused registration at a GP practice if you can’t provide identification or proof of address.

Anybody in Scotland can access primary care services at a GP practice without charge.

You will be asked to complete registration forms for each individual wishing to register alongside a short questionnaire. On completion of both forms over 16’s are required to produce two forms of identification, one of which must be photographic, along with confirmation of address e.g.utility bill.

As a Practice, we assume that patients have consented to the disclosure of personal health information, to the extent that it is necessary for the purposes of the operational management of the Practice and related facilities concerned with the provision of care.

The Practice complies with the Data Protection Act 2018.

Registering as a Temporary Resident

You can only register with a GP practice if you’re staying in the area for more than 3 months. If you’re not, you may still be treated but as a Temporary Resident.

You can also use the Access to Healthcare. This is a GP registration card to help you register with a GP Practice. The card gives information on your rights to accessing healthcare. You can find the card on the Health Literacy Place website.

If you have run out of medication while away from home, and are registered with a Scottish GP practice, you can attend any community pharmacy who will be able to help you without being seen by a GP.

For more information on being treated as a Temporary Resident please contact your nearest GP practice.

Accessing someone else’s information

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us.

To requests proxy access, please collect a proxy access form from reception from 9am to 5pm.

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.
